Ischemic stroke results from occlusion of a cerebral artery and irreversible injury to the brain, and it is the leading cause of disability and the fifth leading cause of death in the USA. There has been recent and unprecedented advancement in the treatment of ischemic stroke patients with the publication of five randomized trials (MR CLEAN, REVASCAT, ESCAPE, SWIFT PRIME, and EXTEND-IA) that demonstrated a marked improvement in patient outcomes following endovascular reperfusion therapy. An essential component in these trials was the use of advanced CT or MR imaging that assessed at a minimum the degree of cerebral infarction and cerebral vascular imaging by CT or MR Angiography. The SWIFT PRIME and EXTEND-IA also included perfusion weighted imaging (PWI) when selecting patients for endovascular therapy, and these two trials demonstrated the best outcomes with functional independence after endovascular treatment in 60% and 71%, respectively.
